DOCARE AOC Travel Grant Application

This is the application for the DOCARE travel grant. Because this grant is funded by the Arizona Osteopathic Charities, only osteopathic medical students from Midwestern University-Glendale (AzCOM) and AT Still University School of Osteopathic Medicine in Arizona (ATSU-SOMA) are eligible to apply.

Two scholarships will be awarded, in the amount of $1000 each. The funds can be applied to any DOCARE global health outreach trip or for a rotation at one of our clinic sites during the period of June 2019 through May 2020. Scholarship winners will be asked to create a blog of their experience to be shared with DOCARE for use in social media and other publications.

Applications will be accepted on a rolling basis through May 2020, or until funds are expended.
